Who in your hospice and palliative care program makes a difference in your community?
Oregon continues to be among the nation’s leaders in hospice care. Oregon families depend on the commitment and compassion of Oregon’s hospice care teams. The Oregon Hospice & Palliative Care Association applauds the work of all the hospice and palliative care teams in Oregon communities.
Each fall, the Oregon Hospice Association celebrates individuals who exemplify the best in quality hospice and palliative care by naming the Hospice & Palliative Care Dream Team.
The Oregon Hospice & Palliative Care Association Board of Directors invites member agencies to submit nominations for staff members whose contributions improve the access to and the quality of hospice and palliative care in your communities. The Oregon Hospice & Palliative Care Association Board will consider your nominations February through August 11 and recognize the 2017 Hospice & Palliative Care Dream Team at the annual Professional Practices Exchange September 24-26 at Eagle Crest Resort in Redmond. Rather than holding an awards dinner at the PPE this year, the Dream Team will be announced at lunch during the conference. Dream Team recipients will receive honorary certificates from the OHPCA Board of Directors.
The Oregon Hospice & Palliative Care Association will name a Hospice & Palliative Care Dream Team member in each of the following disciplines:
- Hospice Aide
- Social Worker
- Chaplain or Spiritual Care Provider
- Professional Staff (for example: Volunteer Coordinator, Bereavement Coordinator, Community Relations staff, Massage Therapist, Music Therapist, Acupuncturist, Dietician, Office staff, IT personnel, etc.)
We invite Oregon Hospice & Palliative Care Association member agencies to submit nominations in no more than two disciplines by completing the attached Dream Team Nomination Form. Applications received by 12 pm, August 11, 2017 will receive consideration.
