This presentation will provide a step-by-step guide on how to conduct an effective IDG\, including documentation to meet the Plan of Care requirement. We will review all aspects of incorporating assessments\, patient visits (both routine and PRN)\, care coordination\, and changes of condition into the Plan of Care. Updating and reviewing the POC per the regulations and what team members should be involved will also be covered.
