The purpose of this 12-session ECHO is to increase the capacity of primary care clinicians and community care teams to provide primary palliative care services.
Topics will include:
- Total Pain Model guidelines for Quality Palliative Care
- Symptom management for serious illness
- Ethical and legal aspects of care/ end of life choices
- Compassionate Community Model: engaging with community-based supports
- Care Conversations: advance care planning and delivering difficult news
