The purpose of this 12-session ECHO is to increase the capacity of primary care clinicians and community care teams to provide primary palliative care services.

Topics will include:

  • Total Pain Model guidelines for Quality Palliative Care
  • Symptom management for serious illness
  • Ethical and legal aspects of care/ end of life choices
  • Compassionate Community Model: engaging with community-based supports
  • Care Conversations: advance care planning and delivering difficult news
