Clinicians provide care in settings where they frequently function autonomously. Responding and recognizing changes in a patient’s condition requires the ability to observe, assess, communicate, and document any deviations from a baseline health assessment and status. When we recognize a change in a patient condition, we can appropriately support the level of care needed. This includes documenting to support continuing hospice and home health services. This presentation will help guide clinicians and team members through recognizing a change in condition, what and how to document, and the appropriate actions to take including different levels of care and service. End-of-life changes will also be discussed.
